Position Summary/Job Duties:

Principle Job Duties:

 

  1. Assist in managing clients lives
  • Set up appointments, transportation and other daily needs
  • Convey concerns to appropriate authorities
  • Grocery shopping for the AFH
  • 2
  • Communication 
  • Clients: Communicate with each resident to understand their wants, needs and concerns
  • AFH Staff:  Train, guide and mentor staff.
  • Nurses:  Give clarification on medications and medication changes including side effects and reactions with other medication; general health questions including notification of changes in physical health, behavior/mental status
  • Doctors:  Request scripts and new medications; notify change of condition in client’s health, clarification of orders
  • Case Managers/Social workers:  Provide updates on changes in clients, general questions regarding clients lives, health and care
  • Pharmacies:  Order medications; request paperwork (scripts, side effects, drug reactions MARs, etc), respond to general medications questions
  1. Paperwork
  • Understand and follow DHS Regulations and all state regulations regarding AFH’s
  • Prepare a new MAR each month to ensure accuracy use cross-referencing as needed (If staff is administering medications)
    • Proper filing and storage of MARS, progress notes, side effects paperwork
  • Add and discontinue medications as prescribed per MD throughout the month
  • Write, fax and file monthly reports to appropriate authorities 
  • Maintain and update resident records, keeping residents personal binders updated including all required forms
  1. Admission
  • Resident ISP: Write and review ISP with residents 
  1. Medications
  • Ensure accuracy of packets sent by pharmacies
  • Return medications as needed, contacting MD/RN for authorization/signed scripts
  • Order supplies for house use
  • Check NARC medications and paperwork for accuracy 
  • Med Room review yearly by pharmacist
  • Medication regimen reviews, yearly by doctor with physical, quarterly psychotropic medication and monthly reviews of PRNs; report issues to residents care team
  1. Facility
  • Maintain a safe, clean facility as stated in DHS regulations
  • Run all emergency drills and maintain corresponding documentation
  • Ensure Fire Extinguishers are serviced yearly

  

  1. Staffing 
  • Have the flexibility to be available as needed

Qualifications:

CNA or CBRF certified is ideal. Training could be provided to the right candidate.

Physical Demands

Light house work like cleaning preparing meals a few times a week.
